March 15 - 2021-2022 Virtual Instruction Program Survey
-
Looking forward to the 2021-2022 school year, Lexington 4 anticipates opening school in August with our normal on-campus instructional program for all students 5 days a week.
Based upon community feedback, some families have indicated that they would prefer an option to attend school via the Virtual Instructional Program (VIP) in the upcoming school year. In order to determine the feasibility of continuing VIP in Lexington 4, we need your input.
Please note that answering this survey does not commit your student to virtual instructional for the upcoming year nor does it guarantee that the district will offer this program for your student.
In order to participate in the 2021-2022 VIP, students must:
- have access to the Internet (hotspots will not be provided).
- be in good academic standing.
- have good attendance.
- have demonstrated success in the current virtual program or be recommended by administration.**IMPORTANT - you will need to complete this form for each student with interest in the virtual program.**
You do not need to complete this survey for students who will attend school on campus.
This survey will close on Friday, March 19, 2021.
